Bleeding Control Kits vs First Aid Kits: What Each One Is For
Is a bleeding control kit the same as a first aid kit?
Short answer: No. A workplace first aid kit built to ANSI/ISEA Z308.1-2021 is stocked for the injuries that happen constantly โ cuts, burns, sprains, eye irritation. A bleeding control kit exists for one low-frequency, high-lethality event: severe arterial bleeding, where a worker can die in minutes. They are layers, not alternatives. The 2021 revision of Z308.1 recognised the distinction directly by requiring an arterial tourniquet in every Class B fill and adding separate guidance on independently packed bleeding control kits for workplaces where that risk genuinely exists.
Bleeding control kits vs first aid kits: what each one is for (2026)
A safety manager who has just read the ANSI kit tables usually arrives at one of two wrong conclusions: that a Class B kit already covers catastrophic bleeding, or that a trauma kit on the wall makes the first aid kit redundant. Neither is right, and the gap between them is where people bleed out waiting for EMS. This reference sets out what each kit is designed to do, what the standard actually requires, the tourniquet specification most buyers get wrong, and how to decide whether your site needs the bleeding-control layer at all.
Why this matters.
Uncontrolled bleeding from a major limb artery can be fatal in a handful of minutes โ faster than a typical EMS response almost anywhere. That is the entire argument for the bleeding-control layer: it is not about better treatment, it is about the window before professional help arrives. Everything a first aid kit does well assumes you have time. This is the case where you do not.

What the standard actually requires โ and what it only suggests
Two different things happened in the 2021 revision, and conflating them causes most of the confusion.
- A tourniquet became a required item in Class B kits. Not Class A. If your site runs Class A fills, the standard has not put a tourniquet in your kit.
- Separately packed bleeding control kits got guidance, not a mandatory fill. Z308.1-2021 advises that where a risk of severe, life-threatening bleeding exists, a dedicated bleeding control kit should be considered in addition to the first aid kit. That is a recommendation, not a required contents table.
And a point that gets lost constantly: neither is an OSHA requirement in the enumerated sense. 29 CFR 1910.151(b) requires adequate first aid supplies to be readily available and a person adequately trained to render first aid. OSHA does not list contents, and Z308.1 is referenced in the standard's non-mandatory appendix as guidance โ it is not incorporated by reference and is not enforceable as a shopping list. What is enforceable is the word "adequate," measured against your own hazard assessment. On a site that cuts, crushes or shears, that word does real work.
The tourniquet specification most buyers get wrong
Z308.1-2021 requires an arterial tourniquet in Class B fills, and the standard is explicit that a venipuncture band โ the thin flat strap used for drawing blood โ does not qualify. This matters because cheap kits and cheap refills have been found carrying exactly that.
The performance envelope the standard describes:
- At least 1.5 inches wide. Narrow straps concentrate force, hurt more and are less effective at occluding arterial flow.
- Effective across limb circumferences of roughly 7 to 33 inches, so one device works on a small forearm and a large thigh.
- A mechanical advantage โ in practice a windlass or ratchet โ because hand tension alone will not stop arterial flow in a thigh.
When you audit a kit, this is the single highest-value thing to check. Pull the tourniquet out, measure the width, and confirm it has a windlass and a securing mechanism. A "tourniquet" line item on a packing list proves nothing.
Does your site need the bleeding-control layer?
This is a hazard-assessment question, not a budget one. Build the capability if any of these are true:
- Powered cutting, shearing, crushing or rotating equipment that a limb can enter โ saws, guillotines, augers, balers, meat processing.
- Glass, sheet metal, or blade handling where a deep laceration is plausible.
- Remote or dispersed work where EMS response exceeds a few minutes โ which is most construction, utilities, agriculture and forestry.
- Public-facing sites with a documented violence or vehicle-incursion risk.
- Any site where your incident history already includes a significant laceration.
Where none of these apply โ a typical office โ the Class A fill plus a trained responder is a defensible position, and adding an IFAK nobody is trained on buys nothing.
Staging: the decision that determines whether it works
A bleeding control kit is a seconds-matter intervention, which makes location more important than contents. Site it at the hazard, not in the first aid room. A kit two minutes' walk away is a kit that arrives after the survivable window has closed. Practical rules:
- Mount at the machine, the vehicle, or the work face โ wherever the arterial risk actually is.
- Make it visibly distinct from the first aid kit so nobody grabs the wrong box under stress.
- Keep it sealed with a tamper indicator so you know at a glance it is complete.
- Train everyone who works in that area, not just the designated first-aider โ the person present is the person who will use it.
- Record and rehearse who calls 911 while the tourniquet goes on. Both happen at once, or neither happens well.
Worked example: a fabrication shop
A 40-person metal fabrication shop runs a shear, two band saws and a press brake, with EMS about eight minutes out. The existing provision is one Class A cabinet in the break room.
The Class A cabinet is the wrong baseline. The injury profile here โ deep lacerations from sheet edges and blade contact โ is what the Class B fill exists for, and Class B brings the splint, the larger eye/skin wash and the arterial tourniquet. Upgrade the cabinet to Class B and site a second one near the shear.
Class B alone still is not the answer. One tourniquet in a break-room cabinet, eight minutes from EMS, sixty feet from the shear, is not a bleeding-control capability. Add two dedicated bleeding control kits โ one at the shear, one at the saw bank โ each with a windlass tourniquet, packing gauze, a pressure bandage, gloves and shears, sealed and signed.
Then do the part that actually saves the life. Train the shop floor, not just the first-aider, and rehearse the split: one person applies pressure and the tourniquet, another calls 911 and meets the ambulance at the door. Check both layers on the same documented cycle as everything else, using the first aid kit inspection checklist.

Train the whole work area rather than only the designated first-aider, because the person standing there is the person who will act.
Should a tourniquet be loosened once applied?
No โ not by a workplace responder. Once an arterial tourniquet is on and bleeding has stopped, it stays on until a clinician removes it. Loosening to "check" restarts the bleeding and can wash out the clot that formed. Write the application time on the device or the patient and hand that information to EMS; that timestamp is what the receiving clinician needs and it is the step most often forgotten.
Do bleeding control supplies expire?
The sterile items do โ packing gauze, hemostatic gauze and pressure bandages carry printed dates, and a breached wrapper voids sterility regardless of date. Tourniquets do not spoil chemically, but webbing, windlass and hook-and-loop degrade with heat, UV and vehicle storage, so inspect and retire on condition. Never return a tourniquet used in training or in a real event to the response kit.
Does the bloodborne pathogens standard apply?
Where employees have occupational exposure to blood or other potentially infectious materials, yes โ and a designated responder expected to control severe bleeding as a collateral duty is squarely in that population. 29 CFR 1910.1030 then brings a written exposure control plan, training, PPE and the hepatitis B vaccination offer. Gloves in the kit are part of the answer, not the whole of it.
