How to Clean Up Blood and Body Fluids Safely: OSHA Bloodborne Pathogen Procedure (2026)

Cleaning up blood or body fluids is a regulated procedure, not a mop job. Under OSHA 29 CFR 1910.1030 the work is governed by universal precautions โ€” every spill is treated as infectious regardless of who left it or how healthy they looked. Here is the nine-step procedure, the PPE order that keeps the hazard off your skin, and the waste and reporting rules that close it out.

Why a blood or body-fluid spill is a regulated cleanup

Hepatitis B can stay infectious on a dry surface for at least a week. You cannot tell by looking at a spill whether it carries HBV, HCV or HIV, and you cannot tell by looking at the person who left it. That is the entire reason the standard is written around universal precautions rather than risk assessment on the spot: the cleanup behaves the same way every time, so nobody has to make a judgement call while standing over it.

Why this matters.
OSHA's bloodborne pathogens standard applies wherever occupational exposure is reasonably anticipated โ€” and that determination has to be made without regard to whether PPE is worn. For building services this is not a hospital question: restroom cleaning, waste handling and spill response are exactly where custodial staff meet blood and sharps. Where the exposure exists, the employer owes an exposure control plan, training, PPE at no cost, and the hepatitis B vaccine within 10 working days of assignment.

What the standard actually requires

The obligations below sit on the employer, not the worker. They are the difference between a compliant cleanup program and a bucket in a closet.

The procedure

Nine steps, in order. The order is the control โ€” most exposures happen when a step is taken out of sequence, usually by wiping before absorbing or by de-gloving before the waste is closed.

  1. Clear the area and put a barrier around it. Move people back before anything else. A blood spill on a hard floor spreads under foot traffic and a slip on it turns one incident into two. Set out wet floor signs or cones at a radius you can actually hold, and keep the route to your supplies clear so you are not walking contaminated back through clean space.
  2. Put PPE on before anything touches anything. Gloves, then gown or coverall, then sealed eye protection.   6. Work clean-to-dirty, edge to centre. Start at the outside of the affected area and work inward, so you are never dragging contamination across ground you have already finished. Change cloths often. Do not return a used cloth to the disinfectant container โ€” that turns your solution into a reservoir.
  7. Bag it as regulated waste and label it. Wet absorbent, used cloths and contaminated PPE from a real spill are regulated waste. They go into a closable, leak-proof container or bag, labeled with the biohazard symbol in fluorescent orange or orange-red, or color-coded red. Double-bag if the outside of the first bag is contaminated. Never compress a bag by hand to make room โ€” that is how people find the sharp they missed.
  8. De-PPE in the right order, then wash. Contaminated first, cleanest last. Gloves off glove-in-glove, gown or coverall rolled away from the body so the outside stays inside, eye protection off by the strap, inner gloves last. Then wash your hands with soap and running water โ€” immediately, not at the end of the shift.
  9. Restore the area, then close the paperwork. Leave signage up until the floor is dry. Restock whatever the kit consumed before the cart goes back, because the next event will not wait for a purchase order. If anyone had skin, eye or mucous-membrane contact, the exposure report starts now โ€” not tomorrow.

Sharps and broken glass

Sharps are the reason this procedure is different from every other cleaning task in the building. A puncture delivers the exposure past every barrier you are wearing, which is why the rule is absolute: nothing sharp is ever picked up by hand, gloved or not. Tongs, forceps, or a dustpan and brush, straight into a puncture-resistant container that closes.

Two habits cause most of the injuries. The first is pressing down on a waste bag to make room, which drives an unseen sharp through the plastic and into a hand. The second is reaching into a bin "just to move it" โ€” restroom and waste-handling routes are where custodial staff most often meet a needle they were not expecting. Route planning and how the cart is set up belong in the same conversation as PPE.

After an exposure incident

An exposure incident is specific: contact with blood or OPIM to the eye, mouth, other mucous membrane, non-intact skin, or through a puncture. If one happens:

  1. Wash or flush immediately. Soap and running water for skin; water or sterile saline for eyes and mucous membranes.
  2. Report it the same day. The clock on post-exposure prophylaxis is short, and a late report can remove the option entirely.
  3. Confidential medical evaluation. The employer must make it available immediately at no cost, including source testing where law permits, baseline bloodwork and prophylaxis where indicated.
  4. Document it. Route the sharps injury into the log, and feed what happened back into the exposure control plan โ€” that annual review is supposed to be driven by real incidents.

A written procedure that nobody has been trained on does not survive its first real event. Common questions

Does OSHA's bloodborne pathogens standard apply to janitors and custodians?

It applies wherever occupational exposure is reasonably anticipated โ€” not automatically to every cleaning job. If part of the role is cleaning restrooms, handling waste that may contain sharps, or responding to spills of blood or other potentially infectious material, the employer has to make that determination and, where exposure is anticipated, cover the worker under 29 CFR 1910.1030. Job title does not decide it; the exposure determination does, and it has to be made without regard to personal protective equipment.

What PPE do you need to clean up a blood spill?

At minimum: fluid-resistant disposable gloves, eye protection that seals against splash, and a fluid-resistant gown or coverall when splashing is possible. Add shoe covers when the spill is on the floor and a face shield when there is any chance of spatter. The standard requires the employer to provide it at no cost to the worker.

Can you use bleach to disinfect a blood spill?

Yes, if it carries an EPA registration for the pathogens involved and you follow the label. Public-health guidance commonly cites a 1:10 dilution of household bleach for large blood spills and 1:100 for surface disinfection after the bulk has been removed. The label's contact time governs โ€” a surface that dries in 30 seconds when the label calls for five minutes has not been disinfected.

What is OPIM?

Other Potentially Infectious Material. Under 1910.1030 it covers semen, vaginal secretions, cerebrospinal, synovial, pleural, pericardial, peritoneal and amniotic fluid, saliva in dental procedures, any body fluid visibly contaminated with blood, and all body fluids where it is difficult or impossible to tell them apart.

Do you have to treat vomit and urine as bloodborne hazards?

Urine, feces, vomit, sweat, tears and nasal secretions are not OPIM under 1910.1030 unless they contain visible blood. They still carry their own hazards โ€” norovirus in particular โ€” which is why the vomit procedure is a separate protocol. When blood is visible in any of them, treat the whole event as a bloodborne cleanup.

How do you pick up broken glass or a needle from a spill?

Never with your hands, even gloved. Use mechanical means โ€” tongs, forceps, a dustpan and brush, or a scoop โ€” and put the item straight into a closable, puncture-resistant, leak-proof container that is labeled or color-coded. Do not recap a needle, and do not press down on the bag to make room.

What counts as regulated waste?

Liquid or semi-liquid blood or OPIM; items caked with it that could release it if compressed; items soaked enough to release blood or OPIM if squeezed; contaminated sharps; and pathological or microbiological waste. Ordinary wiping cloths with a small dried stain generally are not โ€” but the wet absorbent from a real spill usually is.

What color does a biohazard bag have to be?

Containers for regulated waste must be labeled with the biohazard symbol in fluorescent orange or orange-red with contrasting lettering, or color-coded red. Red bagging substitutes for the label. State and local waste rules can be stricter than the federal floor.

What is the correct order to remove contaminated PPE?

Contaminated first, cleanest last, and never touch the outside with bare skin. Gloves come off first (glove-in-glove), then gown or coverall rolled away from the body, then eye protection by the strap, then the second pair of gloves if double-gloved. Wash hands with soap and running water immediately afterward.

Do you need to wash your hands if you wore gloves?

Yes. The standard requires washing hands immediately, or as soon as feasible, after removing gloves or other PPE. Gloves fail in ways you cannot see, and contamination transfers during removal. Hand sanitizer is a stopgap when running water is not available, not a replacement for it.

What should you do immediately after a needlestick or a splash to the eye?

Wash the exposed skin with soap and water, or flush a mucous membrane with water or saline, then report it straight away. The employer has to make a confidential medical evaluation available immediately, including source-patient testing where permitted, baseline bloodwork and post-exposure prophylaxis where indicated. Is the hepatitis B vaccine required, and who pays for it?

The employer must offer it at no cost to every worker with occupational exposure, within 10 working days of initial assignment, after training. The worker can decline โ€” but has to sign the declination form, and can ask for the vaccine later at any point while still covered.

How often is bloodborne pathogens training required?

At the time of initial assignment to a job with occupational exposure, and at least annually after that. Additional training is required when a change in tasks or procedures affects exposure. Training records are kept for three years; medical records for the duration of employment plus 30 years.

Can a general-purpose cleaner be used instead of a disinfectant?

No. Cleaning removes soil; disinfection kills what is left. A blood cleanup needs both, in that order โ€” an EPA-registered disinfectant cannot work through a layer of organic material, which is why the bulk absorb and the wipe-down come before the disinfectant goes down.

Who is allowed to do the cleanup โ€” any employee?

Only someone trained and covered under the exposure control plan, using provided PPE. Handing a mop to whoever is nearest is the most common way this goes wrong. If nobody on shift is trained and equipped, the correct action is to restrict the area and call someone who is.
